Prescription Drug Coverage Available
With Tufts Medicare Preferred HMO, you can choose to have both medical and prescription drug coverage in one plan. We help make it simple... and affordable.
Although you have the option to receive medical coverage only, with Tufts Medicare Preferred HMO, you can add Medicare Part D prescription drug coverage. You may choose from two prescription drug options, Rx and Rx Plus.
Doctor and Hospital Network
With Tufts Medicare Preferred HMO, you will select a Primary Care Physician (PCP) from our network of participating physicians in your area.
Your PCP will coordinate all of your routine care and refer you to plan specialists and other health care providers within his/her referral circle when needed. To be covered, you must receive all routine care from plan providers within your Primary Care Physician's referral circle. If you obtain routine care from a provider outside of your PCP's referral circle neither Medicare nor Tufts Medicare Preferred will be responsible for the costs. In most cases, you will be referred to those specialists, skilled nursing facilities and other providers and suppliers that are part of your Primary Care Physicians referral circle.
Who is eligible for Tufts Health Plan Medicare Preferred HMO?
To join this plan, you must:
- Be eligible for Medicare on the basis of age or disability.
- Be entitled to Medicare benefits under Part A and enrolled in Part B.
- Continue to pay Medicare Part B premiums, if not otherwise paid for under Medicaid or by another third party.
- Live in the Tufts Medicare Preferred HMO Service Area.
- Not currently have End-Stage Renal Disease (ESRD).
NOTE: If you do not need regular dialysis, or have had a successful kidney transplant, you are eligible by supplying a doctor's note or record with your enrollment form.
Plan Service Area
To join Tufts Medicare Preferred HMO, you must live in Barnstable, Bristol+, Essex, Hampden, Hampshire, Middlesex, Norfolk, Plymouth+, Suffolk, or Worcester county.
+Partial counties: If you live in one of the following zip codes, you live outside the service area and are not eligible to enroll in our HMO plan: Bristol County - 02715, 02718, 02764, 02779, 02780, 02783. Plymouth County: 02344, 02346, 02347, 02348, 02349.
