Group HMO Plans - A good fit:
If you want medical and prescription drug coverage in one plan with extra benefits like routine vision and hearing exams, $150 per year towards eyewear or contact lenses (but not both), $150 per year towards fitness club membership, instructional fitness classes, and/or nutritional counseling and more.
