How To Make Your Next Doctor Office Visit A Success  
The best way to have a successful doctor office visit is to prepare in advance. By preparing your health information and any questions or concerns you may have before your next appointment, you are more likely to feel satisfied with your doctor visit. This easy-to-use guide can help you get the most from your next appointment by noting helpful things you can do before, during, and after your visit.  
 
Before the Visit
In preparation for your next doctor visit, you may find it helpful to fill out the information sheet provided below and bring it with you. During doctor visits, many people forget to ask important questions or find it difficult to recall all of their health information. By writing down your questions and listing any medical conditions before going to the doctor, you are more likely to have a satisfying visit.
 
Completing the information sheet below and bringing it with you will provide your doctor with important details about your medical conditions and help you to remember all of your questions.

During the Visit
Tell your doctor what you wrote down on the “Current Medical Conditions” section on the ‘Doctor Visit Information Sheet’ and review it with him. Then discuss any of the health concerns you added to the “Notes To Make Before Your Visit” section.

You may also want to write down any information your doctor gives you during your visit. Included in the ‘Doctor Visit Information Sheet’ are some common topics that are discussed during an office visit.

After the Visit
Remember to follow these important steps after your visit:

  • Follow your doctor's advice. If you can't remember what to do, call the doctor's office and ask again.
  • Tell your doctor if you feel worse, have additional problems, or have bad side effects from medications your doctor told you to take.
  • Keep return visit appointments. If you need to reschedule, call your doctor's office at least 24 hours ahead of time.
