Stay Fit and Save On Weight Management Programs!

You can get reimbursed annually (up to $150) toward the fees you pay for weight management programs.1
This benefit is in addition to your Wellness benefit. Plus you can save even more on weight management programs by using your Preferred Extras2!
How it works:
Once you’ve participated in your weight loss program, keep your payment receipt—you’ll need it to request reimbursement.
- When you’re ready to ask for reimbursement, fill out the Weight Management Benefit Reimbursement Form available at thpmp.org/forms.
- You can submit several forms during the year that total the amount available for reimbursement, or you can send in one form for the total reimbursement amount.

1$150 is the total reimbursement amount each year (Jan. 1–Dec. 31). This benefit does not cover costs for pre-packaged meals/foods, books, videos, scales, or other items or supplies.
