1Non-emergency. Exceptions may apply. Non-medical rides limited to 2 round trips per month, 20 miles each way of trip. Members must use approved vendor to access benefit.
2Your Instant Savings card can be used to purchase eligible items from participating retailers and plan-approved online stores. Quarterly credit of $115 is for the purchase of eligible Medicare-approved OTC items. Quarterly credit of $235 is for the purchase of eligible Medicaid-approved OTC items as well as eligible groceries. Unused balances at the end of a calendar quarter will not roll over to the next quarter. Under certain circumstances, items may be covered under your Medicare Part B or Part D benefit.
3$300 available toward the full retail price (not sale price) for eyeglasses (lenses, frames, or a combination) or contact lenses from a provider in the EyeMed Vision Care network or up to $180 from a store not in the EyeMed Vision Care network.
