Your care
Tufts Health One Care (D-SNP) is a plan that simplifies your health care by providing a dedicated care coordinator and a care team that help you get the right health care services.
Your care plan
Your Cityblock Health care coordinator works directly with you and your care team to create an individualized care plan (ICP) that details your health needs and how your care team will help address those needs.
Your care team
Your care team makes sure you get regular checkups, preventive care and treatment for ongoing conditions or concerns.
Over-the-Counter Benefits
With your over-the-counter (OTC) benefit, Tufts Health One Care (D-SNP) members get an Instant Savings card to spend every calendar quarter on grocery, household, and OTC health-related items at participating retailers and plan-approved online stores.
