Your Member ID Card

New Member ID Cards for 2026

All Tufts Health One Care plan members should have received a new member ID number and ID card in the mail starting October of 2025 to be used starting January 1, 2026. Your new ID card has a gold bar on the bottom of the card. Please also note your new member ID number listed on your new ID card. This card has important information for you and your providers.

Requesting a new ID card

You can also request a new member ID card with your member portal, our self-service tool. With your member portal, you can do things like keep track of your medical history as well as:

• Choose or change your primary care provider (PCP)
• Update your contact information
