Broker Application Submission During Scheduled Maintenance Periods

Brokers will continue to be able to submit applications during all upcoming maintenance periods via the following methods:

  1. Brokers who have a Personalized URL (PURL) will be able to use this method to submit applications during these maintenance times. If you do not have a PURL, please contact Medicare Broker Support for assistance at 833-984-2387 or medicarebrokersupport@point32health.org.
  2. Complete a paper application, downloadable here, and submit it via a 
    secure email to Medicare Broker Support at medicarebrokersupport@point32health.org
  3. Fax the completed application to Tufts Health Plan at 617-972-9475
