What does a Care Manager do?
As a Tufts Health Plan Senior Care Options member, you automatically get a dedicated Care Manager to make sure you get the care and support you need to stay healthy. Care Managers are nurses that work closely with your doctor to help guide you through the health care system, improve your health, and more. This includes helping you manage your medications and any health conditions you may have, coordinating rides to your medical appointments, helping you get services like home-delivered meals and transportation if you need it, and much more!

From day one, your Care Manager works hard to provide all the personalized care, attention, and support you or your loved ones deserve. Your Care Manager will make sure you understand your care options and help you get the services and care you need.
Knowledge and support
Health care can be overwhelming. Your Care Manager will help you access your benefits and explain how your Tufts Health Plan Senior Care Options plan works. Your Care Manager can answer any questions you have about your care or using your plan.
Manage your chronic condition
If you have a condition such as diabetes, heart failure, or lung disease, your Care Manager can help you develop health goals, and help ensure you receive the right services and resources. Your Care Manager will work with your doctors to design a care plan specific to your needs.
Recover at home
If you are hospitalized, your Care Manager can help make your transition home easier. Your Care Manager will make sure you receive the right services to help prevent return trips to the hospital, including getting care at home if you need it and making follow-up appointments with your doctor.
