How to use your Evidence of Coverage (EOC).
Your EOB shows you the costs for any medical services you received from the previous month, but it’s not a bill. You don’t need to send any payments or take any action. It helps you track your medical costs and shows you how expenses are divided between your doctor, your health plan, and your copay.
If you’re 65 or older (or under 65 and have a disability or other qualifying condition), you’re eligible to receive health insurance through Medicare. Original Medicare, offered by the federal government, includes Part A (hospital insurance) and Part B (medical insurance).
When you join an HMO plan, you select a doctor to be your primary care physician (PCP). Your PCP will provide your routine care, preventive care, and treatment for common illnesses. Your PCP is also responsible for coordinating or overseeing your care.
